Certified Peer Support Specialist
The Peer Support Specialist is responsible for Diversion and In Reach activities targeting the Special Healthcare Population with Serious and Persistent Mental Illness (SPMI). This role involves working in various settings such as Adult Care Homes, State Hospitals, Correctional Institutions, and community settings. The Peer Support Specialist educates about community resources, services, supports, and permanent supportive housing, ensuring members receive comprehensive care that includes physical health, behavioral health, and social determinants of health.

Essential Functions
Engage members to provide education on supportive housing and community resources.
Collaborate with the care team to facilitate transition planning and community integration.
Conduct assessments to identify service needs and coordinate care across multiple providers.
Utilize person-centered planning to address individual needs and support members in navigating the healthcare and social support systems.
Document activities accurately in The Company Connections care management platform and ensure compliance with all regulatory requirements.
Primary Objectives
Educate and engage members in comprehensive planning and transitioning to community living.
Monitor and ensure service delivery and compliance with state standards and Medicaid regulations.
Actively participate in team meetings and transition planning activities to ensure holistic member care and achievement of positive health outcomes.
Required Experience and Education
At least one year of paid experience in the Health and Human Services field, working directly with individuals experiencing serious mental illness, substance use disorders, or intellectual/developmental disabilities. Alternatively, personal or familial lived experience with a behavioral health condition, coupled with navigation of the North Carolina Medicaid delivery system.
High school diploma or GED.
Required: Certified Peer Support Specialist Certification - NCCPSS

Proficiency in Microsoft Office suite and adaptability to new technologies.
Ability to organize multiple tasks and manage projects effectively.
Strong problem-solving skills and the ability to resolve issues with minimal assistance.
Accurate administrative judgment to maintain quality of care and program success.
Efficient handling of documentation to meet accreditation standards and support data-driven outcomes.
Role Details/Working Conditions
May involve transportation of members to community settings as part of community reintegration planning. Adherence to all applicable NCQA, EQR, and DOJ requirements.

8 hour shift, 40 hours per week
